Provinces make their own rules
A province is held by a landholder, and how they govern is up to them: running it alone, appointing a council, holding elections. Each sets its own rules, taxes and culture, so picking one is really a choice about how you want to play. Any player can ask to start a province, once the administration is confident you can run one.
